Support for Persons with Disabilities (S2021)
United for Humanity, in line with human values, social justice, and individual dignity, has launched a special program titled Support for Individuals with Disabilities (Program Code: S2021). This program is entirely free and relies solely on humanitarian resources. It is designed for individuals with disabilities who live in critical conditions or without adequate support.
Objectives and Key Focus Areas of the Program:
Providing humanitarian financial assistance to cover basic needs such as food, housing, medicine, and medical supplies
Providing medical and rehabilitation services, including assistive devices like wheelchairs, walkers, prosthetics, or devices for hearing and vision
Offering psychosocial counseling and social integration services to improve individual and family quality of life
Creating opportunities for social and cultural participation to reduce isolation and enhance self-esteem
Access to legal and administrative counseling when necessary, especially for refugees or individuals without legal documentation
Eligibility for Services:
This program is available only to individuals who do not receive any form of government or private support.
Submission of valid medical documentation and an official disability or physical/mental disorder diagnosis is required.
The applicant’s social and economic status will be assessed based on submitted documents and an interview.
Priority will be given to individuals living in critical areas, refugee camps, or without family support.
Required Documents and Forms: To submit an official application, the applicant must complete and send the following two forms simultaneously:
Main Disability Support Form – Code S2021
Special Conditions Assessment Form – Number H4460
Both forms must be completed, and applications lacking either of the forms will not proceed to the review stage.
How to Submit an Application: Please download both forms from the [Humanitarian Forms] section, complete them accurately, and send them along with medical and identification documents via our official email address.
If needed, applicants will be invited for an interview or further information completion.
The evaluation result will be sent to the applicant’s email within a maximum of 14 business days.
